# Add a form to your Ghost site

**Works through the script embed.** Checked on self-hosted Ghost 6.73.0, November 2026. No integration needed.

Your form goes in an **HTML** card, inside a page or a post. Ghost has no form of its own, so this is all it takes.

## Before you start

[Publish your form](/guides/add-a-form/), then on the **Publish** tab, choose **Embed** and copy the code. It looks like this,
with your own form's address:

```html
<script
  src="https://forms.formerie.com/embed.js"
  data-formerie-form="fm_t77txbvg43nmar"
></script>
```

## Add it to a page

1. In Ghost Admin, go to **Pages**, and create a page or open one.
2. On an empty line of the body, type `/html`, then press Enter on **HTML**.
3. Paste your code in the card.
4. Click outside the card, for example on the title.
5. Choose **Publish**, then **Continue, final review**, then **Publish page, right now**.

## Check it arrived

Open the published page, fill out the form and submit it. It says it was received, and the answers show on the form's
**Submissions** tab.

That's a real submission, so your recipients get the email. On your own computer, at an address with a port such as
`http://localhost:8080`, it's stored as a test instead, and the form says so. To try the form on any other address
without reaching them, use a [try link](/guides/test-your-form/#share-a-try-link).

## What can go wrong

| What you see | Why | What to do |
| --- | --- | --- |
| `/html` stays as text | The menu only opens on an empty line | Start a new line, and type it again |
| The code shows on the page as text | It went in a normal paragraph, not a card | Delete it, and add the **HTML** card |

## Limits

- It needs JavaScript in the visitor's browser.
- The form takes the width of your theme's content column. This was checked on the default theme, Source.
- A form has one language. For a second language, make a second form and use it on that language's page.
- Ghost(Pro) and email newsletters weren't checked. Don't count on a form inside an email: use a link to the page.
